• Copy of Passport Bio—Data page.
• Birth Certificate.
• Letter addressed to the Honourable Minister for Labour, Public Service Reform, Social Partnership, Entrepreneurship and Small Business Development requesting citizenship and stating reason for Renunciation.
• Copy of Declaration of Renunciation.
• Receipt of payment of $100 processing fee (from Government Treasury).
• $20.00 adhesive stamps for application form.
