Instruction for completing form

Please complete in BLOCK LETTERS. The Cooperative Division is collecting data on this form in accordance to the Cooperatives Societies Act No.2 of 2011.
Pursuant to the Cooperative Societies Act, a cooperative society shall make available at all reasonable times at its registered office the following:

• Copies of all notices of directors and notices of change of directors
• A register of its directors setting out the names, addresses and occupations of all persons who are or have been directors of the co-operative society with the dates on which each person became or ceased to be a director. The Registrar may, during the normal business hours of a cooperative society, observe practices, monitor operations and inspect or authorize the inspection of the cooperative society, including any of the records listed in section 21(2).
