Instruction for completing form

REGISTRATION GUIDELINES

The Cooperative Division is collecting data on this form in accordance to the Cooperatives Societies Act No.2 of 2011.

In keeping with Section 12 sub-section (2) of the Act and Regulation 6, this application must be submitted with all supporting documentation listed at the end of the form and a non-refundable fee.

  • The form should be completed in writing, using BLACK or BLUE ink and BLOCK letters
  • Application Fee of twenty-five (XCD25.00) Dollars
  • Any information provided on additional sheets must be signed and dated.
  • Where there are question which is NOT APPLICABLE, please write “N/A” beside the question
  • All dates must be completed in the form: (D/M/Y)
  • Question left unanswered or which do not disclose all information required will result in this application being deemed as “incomplete”. This will affect the Registrar’s assessment and may delay the time to respond.

Checklist

The following documents are enclosed (please tick in the box):

  • Three (3) copies of proposed by laws of the cooperative
  • Two (2) copies of business plan
  • Two (2) copies of a proof of a bank account
  • Two (2) copies of coop form no.100: establishment of steering committee (with identification numbers and Signatures)
  • The sum of xcd_____________being the prescribed fee.
