RENEWALS (WORK PERMIT)

OTHER PARTICULARS TO BE SUPPLIED WITH RESIDENCE/WORK

APPLICATION FORM (L.I. 01 & 07)

All Non-Nationals above the age of 18 years must complete an application form. Applicants must in addition supply the following particulars with this form:

• One passport-size photograph.
• Letter from employer (sponsor), addressed to the Hon. Minister, accepting responsibility for the employee and stating duration and type of employment including wage to be paid. (Letter on behalf of unskilled workers must include an hourly rate to be paid).
• Photocopy of passport bio data page.
• Valid passport (passport must has at least six (6) months validity)
• A statement from the Commissioner of Police. (A Police Record within the last 6 months from issuance.)

Copy of last Work Permit.

• Photocopy of Treasury receipt as proof of payment of application fee.
• Application processing fee: EC$800.00 (Non-CARICOM) or EC$250.00 (CARICOM Members).

If self-employed please include a copy of the following:

• A Bank Statement (Must provide six months average balance of the account at that institution)
•  Statement of Particulars along with Certificate of Registration of Business
• Certificate of Good Standing
• In case of farming: Proof of ownership of land or lease agreement and a copy of Farmer’s Produce Sellers License Card
